How do I get a renewal for my medications?
I left some forms to be filled out by my physicians, when will they be ready?

Medication renewals

 

Before your appointment, please always check the number of refills left on your medications and let your physician know should you need any refills.  Prescriptions should usually be renewed during an appointment.  Please do not wait until you only have 1 or 2 pills left before contacting us.  It is your responsibility to make an appointment before your prescription expires.

Forms

When you give us forms to complete, please ensure that you have always filled out your section first.  Almost all forms will require, at a minimum, your signature authorizing your physician to complete the form.

 

Please do not wait until the last minute to give us forms!   We try to complete forms in a timely manner and are usually able to complete them within 14 to 21 days.  Please note that as per our College, we have up to 45 days to complete forms.

Please note that the completion of most forms is not covered by OHIP or RAMQ.  For more information regarding fees, please visit the uninsured services section.

Why does the receptionist  ask me for the reason for my visit when I call to book an appointment?

We need to know the nature of your medical problem in order to schedule you properly and have the nurse do the appropriate vital signs before your appointment with the physician.

Missed Appointments and Late Arrivals


We do our best to be on time for our patients.  If you arrive more than 5 minutes late for your appointment, we will not be able to see you as planned.  Appointments that are missed or canceled with less than 24h notice are charged as per the guidelines set out by the Ontario Medical Association.

Services Insured by Health Insurance

The Ontario Health Insurance Plan requires us to check your health card at each and every visit to our clinic. If you show up without a valid health card, you will be required to pay for your visit.  Following this, you can get reimbursed if you get a valid OHIP card in a timely manner.

What are your openings hours?
If the clinic is closed...  where can I get help?

We are open Monday to Thursday from 8AM to 8PM, Friday from 8AM to 4PM and Saturday from 9AM to noon.  We are closed on Sundays and on statutory holidays.

 

If the clinic is closed, please contact Telehealth Ontario at 1-866-797-0000.

Telehealth Ontario is a free, confidential service you can call to get health advice or information. A Registered Nurse will take your call 24 hours a day, seven days a week. Assistance is available in more than 300 languages.
